Since December 2002, the NSW Ombudsman has had responsibility for reviewing the deaths of people with a disability in care and certain children. This is the first annual report on reviewable deaths. It documents our review of 137 reviewable child deaths and 110 reviewable disability deaths.
We acknowledge the traditional custodians of the land on which we work and pay our respects to all Elders past and present, and to the children of today who are the Elders of the future.
